I think going back to a 58 is a good idea. Try to duplicate the old bike as
much as possible.

I'd take some time off. I can't really imagine what you're going through
now as I've never had that kind of pain from riding. Get yourself right and
then let them refit you on the new frame. If you're hurting now, then you
won't be able to know if the new fit works. So don't rush this unless you
want to draw out the awful experience even more.
