how can i fix the gears on my bike?

I bought a bike about a year ago and my gears wont grasp right. first, when i shift into first, the gear fall right down into second and i can't stop it. second, my gears wont shift to the riht position. when my bike is in 1st, it is in 3rd or 2nd and when im in 7, 6, or 5 gear, im in 4th so i need help

It's either worn out and needs to be replaced. Or the derailer needs to be adjusted.
